Victorian Trades Hall survey on workplace injuries

The Victorian Trades Hall Council Return to Work Unit and the NTEU Victorian Division are conducting a survey about workplace injury and returning to work after an injury.

The aim of the survey is for the NTEU and the RTW Unit to gain a better understanding about your workplace in relation to workers compensation and return to work after a workplace injury.  In particular we are keen to learn more about injured workers’ experience.

You can download the survey here. Please print it out and return it to the address on the last page of the survey.

We would very much appreciate your help in completing the survey and return this to the address on the last page of the survey. The information you provide us will be treated with utmost confidentiality.

As stated in the survey the RTW Unit research officer also welcomes your phone call if you prefer to complete the survey over the telephone or in person.
